Examination Fee - $30.00

      * This clinic is open only to TOY dogs.
      * This is a WELL DOG eye clinic.
                 Dogs with obvious eye disease not accepted.
       * Open to all TOY dogs whether or not they are entered in the show.
       * THE NUMBER OF DOGS TO BE EXAMINED IS LIMITED TO 75.
       * Pre-registrations will be accepted until May 18^th using the form below
             (or a copy) for each dog to be examined. Call for availability after May 18th.
        * Additional registrations will be taken the day of the show
                    if space is available.
        * Examinations will begin at 9:00 AM. Entered dogs will be scheduled after
                their judging times if at all possible. Schedule will be posted at the
                 show site.
        * Bring AKC registration or litter number, birth date and copy of prior
                ophthalmologic examination (if any) for CERF forms.
